1.Original passport or National ID.
2.In case of a visit by substitute, notarized Power of Attorney (if the applicant is in Korea, it must be notarized in Korea) & the substitute’s NID card or passport.
3.All documents should be original in principle. If the document is a photocopy or not from the government, notarization by lawyer is required.
4.Contact information of notary public (name of notary office, phone number and address).
5.An application form with all items completed.
6.Fee USD$4 per each copy.
7.Official seal(s) from the related government organization (e.g. in case of academic transcript/certificate, seals from the Ministry of Education and Ministry of Foreign Affairs are required).
* Academic documents which you want to be notarized should be attested by Foreign Ministry , Education Ministry and Lawyer attestation.
